THE FOLLOWING FORM MUST BE COMPLETE BY AUG 10th CAMP AND UPLOADED TO  TEAMSNAP IN ORDER TO PARTICIPATE IN OUR PROGRAM.                                Medical Clearance Form click to print. This document is a Medical Release Form not to be confused with a physical, that must be printed and then signed or stamped by your child's physician. Your child's physical documents do not substitute for this form, as this is a release to participate in our program. Form must be dated after Jan 1,2024.

These documents are viewable at the links below and can be printed if desired, but we will not be collecting them. Please make sure your cheerleader reads and understands the expectations in each contract as consequences of some violations may result in removal from the team.

CAMP AND TUMBLING THE FOLLOWING WAIVERS NEED TO BE PRINTED AND TURNED IN AT CAMP ON AUG 10th       

Camp Waiver Click to print and bring to camp on Aug 10th. Must be on file with your parent rep before you can tumble.

Tumbling Waiver click to print only if your child plans to tumble at camp or games and bring with you on Aug 10th. Note: we do not teach or spot tumbling skills, so list ONLY those skills your daughter has mastered.
